Appointment Pre-Screening 

 

Before booking/showing up for your scheduled appointment you are required to go through the following screening questions and submit them. My salon studio is in my home therefore my family is more at risk and exposed. Please be honest and reschedule or wait to schedule if you answer yes to any of the following questions:

 

 

  • Do I – or anyone I live with - have a fever?

  • Do I -or anyone I live with -have chills, a cough, shortness of breath, runny nose, loss of taste or smell, sore throat, or any other unusual symptoms.

  • Have I traveled in our out of the country or been in a large group gathering in the last 14 days?

  • Have I traveled by air, bus or train in the last 14 days?

  • Have I been in contact with someone who has tested positive for COVID-19?

  • Have I tested positive for COVID-19?

  • Am I or anyone in my household awaiting results of a COVID-19 test?

  • Do I work in an environment where I am at high risk of being exposed to COVID-19 (ex. hospitals, clinics, Dr. Office)

If the answer is yes to any of the above questions, please hold off on booking or contact me to reschedule your appointment. 

* I will have to turn anyone away that appears to be symptomatic 


What to expect when you arrive for your appointment:

  • In order to meet my capacity requirements at this time, please do not bring anyone with you to your appointment. We need to maintain a 1:1 ratio for all services.

  • You MUST WEAR A MASK for the entire visit!  I will wear a mask and possibly a face shield.  – this is a mandate by the State of Minnesota.

  • At the front door I will have hand sanitizer and a pre-screening self-assessment that must be filled out and signed before entry! 

  • Payment forms accepted: Credit Card, Check, Zelle, Exact cash.

  • I will wear gloves when appropriate.

  • I have reserved time to clean and disinfect all implements and service areas between each client

  • I will have open windows for fresh air when possible.

  • I have a clean or disposable cape for each client.

  • I have hand sanitizer available at my station.

  • I will wash my hands before and after services.

  • I will be taking my temperature, monitoring myself for any symptoms and will self-quarantine if needed.

  • I have been certified in the latest Covid-19 sanitation for my industry.

know this will be different then your typical visit with me, I appreciate your understanding and patience as we navigate through this together
